Oramorph Liquid Morphine 2mgml is an immediate-release oral solution of morphine sulfate, a full mu-opioid receptor agonist. It is indicated for acute and chronic pain severe enough to require an opioid analgesic when alternative treatments are inadequate. The 2mg/ml strength is typically for opioid-tolerant patients. Onset: 30–60 minutes; duration: 4–6 hours. Morphine is metabolized to morphine-6-glucuronide (M6G), an active metabolite that accumulates in renal failure, causing prolonged sedation and respiratory depression. Risks: tolerance, dependence, constipation, nausea, and fatal overdose with alcohol or benzodiazepines. Not for opioid-naïve patients.
